At Little River Healthcare Family Care Center Long Term Care, we truly value our referral partners. We strive to maintain the highest quality of service for your patients throughout their healthcare transitions. We believe that during these transitions, patients with complex medical needs, primarily older patients, are at risk for poorer outcomes due to medication errors and other errors of communication among healthcare providers and between providers and patients/family caregivers. Poorly managed transitions can result in important elements of the care plan slipping through the cracks, often resulting in hospital re-admissions. We want to avoid re-admissions for our transitional geriatric patients. Our goal is to maintain the proper level of care for all of our patients and minimize the stress for those patients and their families/caregivers as they transition to a new healthcare facility or to their home. After receiving a patient referral from you, we:

  • Review medical status and current diagnoses of each geriatric patient
  • Assist patients in managing and obtaining prescription medications
  • Evaluate patient conditions to ensure satisfactory progress
  • Work with you and your patient to verify insurance and paperwork
  • Provide education and assistance for family members and caregivers
  • Assist in long-term care planning


We partner with several hospitals, medical centers, home health agencies and skilled nursing facilities across Central Texas. The goal of these partnerships is to help manage and treat healthcare conditions in the home before symptoms progress to a level which requires emergency services.
